Oral Hygiene & Caries Prevention

Kauko Mäkinen

Ph.D., Professor Emeritus — University of Turku, Finland

Professor Kauko K. Mäkinen is one of the most influential figures in the history of preventive dentistry. Over a career spanning more than four decades at the University of Turku in Finland, he conducted the groundbreaking research that established xylitol—a naturally occurring sugar alcohol—as a powerful weapon against dental caries.

His landmark studies, beginning in the 1970s, demonstrated that xylitol not only does not promote tooth decay (unlike sucrose), but actively inhibits the growth of Streptococcus mutans, the primary bacterium responsible for dental caries. This was a paradigm shift: a sweetener that could actually protect teeth.

Mäkinen's clinical trials, including the famous "Turku Sugar Studies," provided the rigorous evidence base that led to the global adoption of xylitol in chewing gums, lozenges, and oral care products. His work showed that regular xylitol use could reduce caries incidence by 30–85% depending on the population studied.

Beyond caries prevention, Mäkinen explored xylitol's broader metabolic properties, including its low glycemic impact and potential benefits for diabetic patients. He also documented how maternal xylitol use could reduce transmission of cavity-causing bacteria to children—a finding with profound public health implications.

His publication "The Rocky Road of Xylitol to Its Clinical Application" chronicles the decades-long journey from laboratory discovery to widespread clinical adoption, highlighting both the scientific rigor and the institutional resistance that characterized this pioneering work.

Professor Mäkinen's research represents the quintessential hygiene innovation: a simple, low-cost, daily-accessible intervention that prevents disease at its source rather than treating it after the fact.
